Woman holding her lower tummy with period cramps

The most effective medicine for period pain for most women is an anti-inflammatory painkiller such as ibuprofen (Brufen) or mefenamic acid (Ponstan), because these reduce the prostaglandins that cause cramps. They work best when started at the first sign of pain or bleeding and taken regularly for the first day or two. Paracetamol (Panadol) is an option if you can’t take anti-inflammatories. A hot water bottle, gentle exercise and a warm bath also help. Pain that stops you from going to school or work, or that keeps getting worse, should be checked by a doctor.

Key points

  • Start the painkiller early: at the first cramp or when bleeding starts, not when pain is severe.
  • Use only one anti-inflammatory at a time: Brufen, Ponstan or Synflex, not two together.
  • Take anti-inflammatories with food. Avoid them with stomach ulcers, kidney problems or some types of asthma.
  • Heat on the lower tummy works about as well as some painkillers for many women.
  • Very painful periods may be caused by endometriosis or fibroids, which are treatable.

Home remedies for period cramps

  • Heat: a hot water bottle or heat pad on the lower tummy or back.
  • Warm bath or shower to relax the muscles.
  • Gentle exercise: walking, stretching or yoga can ease cramps.
  • Light massage of the lower tummy in circular movements.
  • Avoid smoking, which is linked to more painful periods.
  • Ginger tea has some evidence of easing period pain.

What causes period pain?

During a period, the womb lining releases prostaglandins, which make the womb muscle tighten to shed its lining. Higher levels cause stronger cramps, often starting just before or when bleeding begins and lasting 1 to 3 days. This “primary” period pain is common in teenagers and young women and often eases with age or after childbirth.

“Secondary” period pain is caused by a condition such as:

  • Endometriosis: tissue like the womb lining grows outside the womb, causing severe period pain, pain during sex or bowel movements, and sometimes difficulty getting pregnant.
  • Fibroids: non-cancerous growths in the womb that can cause heavy, painful periods.
  • Adenomyosis and pelvic inflammatory disease (infection).

Longer-term options

If painkillers don’t control your pain, a doctor may suggest hormonal treatments such as the combined contraceptive pill, a progestogen-only option or a hormonal coil, which often make periods lighter and less painful. These need a doctor’s assessment.

Frequently asked questions

Which is the best medicine for period pain?

For most women, an anti-inflammatory such as ibuprofen (Brufen) or mefenamic acid (Ponstan), started early. Paracetamol is an alternative if anti-inflammatories aren’t suitable.

Is Ponstan good for period pain?

Yes. Mefenamic acid (Ponstan) is commonly used for period pain and may reduce heavy bleeding too. Take it with food and don’t combine it with other anti-inflammatories.

When should I take a painkiller for period cramps?

At the first sign of cramps or when bleeding starts, then regularly at the label interval for the first day or two.

Can I take Panadol and Brufen together for period pain?

Some adults use them together within the label limits for each, because they work differently. Ask a pharmacist first if you have other health conditions.

Is it normal to have very painful periods?

Mild to moderate cramps are common. Pain severe enough to stop your daily life isn’t something to accept. See a doctor to check for endometriosis or fibroids.

Do hot water bottles really help period pain?

Yes. Heat on the lower tummy relaxes the womb muscles, and studies show it can ease cramps as well as some painkillers.
