The safest cough medicine for kids is usually not a cough syrup at all. Saline nose drops, plenty of fluids, honey for children over 1 year, and paracetamol or ibuprofen for fever work better and more safely. Over-the-counter cough and cold syrups containing dextromethorphan, antihistamines or decongestants should not be given to children under 6. A child’s cough from a cold usually settles within 3 weeks.

Key points

  • Under 1 year: no cough syrups and no honey. Use saline drops, feeds and a doctor’s advice.
  • 1 to 6 years: honey in warm water, saline drops and fever medicine only. Avoid cough and cold syrups.
  • 6 to 12 years: cough syrups only on a pharmacist’s or doctor’s advice, in the dose for the child’s age.
  • Always dose fever medicine by the child’s age and weight, using the syringe or cup in the box.
  • Fast breathing, chest pulling in, blue lips or a baby under 3 months with fever needs a doctor straight away.

Why most cough syrups aren’t recommended for young children

Studies have not shown that cough and cold syrups help young children get better faster, and they can cause side effects such as drowsiness, restlessness, fast heartbeat and, rarely, serious reactions from accidental overdose. For this reason the UK medicines regulator advises that products containing these ingredients should not be used in children under 6:

  • Cough suppressants: dextromethorphan, pholcodine
  • Antihistamines: chlorphenamine, diphenhydramine, promethazine, triprolidine
  • Decongestants: ephedrine, pseudoephedrine, phenylephrine
  • Expectorants: guaifenesin

Many popular syrups in Pakistan, including those labelled “DM” or for “cough and cold”, contain one or more of these. Always check the ingredients on the box.

Home care that helps a child’s cough

  1. Honey (over 1 year only): half to one teaspoon, alone or in warm water, before bed. It soothes the throat and can reduce night-time coughing.
  2. Saline drops: a few drops in each nostril, then gently clear the nose. This helps babies feed and sleep.
  3. Fluids: keep offering water, milk, soup or ORS so mucus stays thin and the child doesn’t get dehydrated.
  4. Raise the head slightly for older children. Babies should always sleep flat on their back.
  5. Keep smoke away: no smoking indoors, and keep children inside on high-smog days in Lahore.

Dosing fever medicine safely

  • Use the dose on the label for your child’s age, and check the strength: 120 mg/5 ml and 250 mg/5 ml paracetamol syrups are different.
  • Always use the syringe or cup that comes with the medicine, not a kitchen spoon.
  • Don’t give paracetamol and a “cold and flu” syrup together unless you’re sure the syrup has no paracetamol in it.
  • Don’t give ibuprofen to a child who is dehydrated, vomiting or not drinking, or who has asthma that worsens with ibuprofen.
  • Never give aspirin to children under 16.

When to see a doctor urgently

Take your child to a doctor or hospital straight away if they:

  • Are breathing very fast, or the skin pulls in between or below the ribs with each breath
  • Make a grunting, wheezing or high-pitched noise when breathing, or have a barking cough
  • Have blue or grey lips, tongue or skin
  • Are under 3 months old with a temperature of 38°C or higher
  • Are very sleepy, floppy or hard to wake
  • Are not drinking, or have far fewer wet nappies than usual
  • Have coughing fits followed by a “whoop”, or vomiting after coughing

Also see a doctor if the cough lasts more than 3 weeks, or if your child has a fever for more than 3 days.

Frequently asked questions

Which cough syrup is safe for a 2-year-old?

Over-the-counter cough and cold syrups with dextromethorphan, antihistamines or decongestants are not recommended under 6. For a 2-year-old, honey in warm water, saline drops, fluids and fever medicine are safer. Ask a doctor if the cough is bad.

Can I give honey to my baby for cough?

Not before 1 year of age, because of the risk of infant botulism. From 1 year, half to one teaspoon of honey can ease a cough.

Are herbal or ivy-leaf syrups safe for children?

Some ivy-leaf syrups and drops are labelled for young children, but the evidence that they help is limited. Check the age on the pack and ask a pharmacist before using one.

How long does a child’s cough last?

A cough after a cold usually settles within 3 weeks. See a doctor if it lasts longer or keeps coming back.

Should I use a nebulizer for my child’s cough?

Only if a doctor prescribes nebulized medicine, usually for wheezing or asthma. Nebulizing plain water or saline at home without advice is not recommended.

When does a child’s cough need antibiotics?

Only when a doctor diagnoses a bacterial infection such as pneumonia. Most children’s coughs are viral and antibiotics won’t help.
