The most common lack of iron symptoms are tiredness, shortness of breath, a fast or pounding heartbeat and pale skin. Other signs of khoon ki kami include headaches, hair loss, brittle or spoon-shaped nails, a sore tongue, restless legs and cravings to eat ice, clay or chalk. Iron deficiency is the most common cause of anaemia in Pakistan, especially in women, pregnant women and children. A blood test confirms it, and it is usually treated with iron tablets or syrup for several months.

Key points

  • Get a blood test (CBC and ferritin) before starting iron, to confirm the cause.
  • Take iron on an empty stomach or with orange juice. Tea, milk and antacids reduce absorption.
  • Black stools and mild constipation are common, harmless side effects.
  • Keep taking iron for about 3 months after your haemoglobin is normal, to refill your stores.
  • In men and in women after menopause, unexplained iron deficiency must be investigated for hidden bleeding.

Shortness of breath on light activityHair loss
Noticeable, fast heartbeat (palpitations)Brittle or spoon-shaped nails
Pale skin, pale inner eyelidsSore tongue, cracks at the corners of the mouth
Feeling coldRestless legs; craving ice, clay (mitti) or chalk

Signs of khoon ki kami in children

Children may be pale, tired, irritable, have a poor appetite, or be slow to grow or learn. Worm infections and a diet heavy in milk but low in iron-rich food are common causes in Pakistan. A child with these signs should see a doctor rather than being given iron without a test.

Common causes

  • Heavy periods: the most common cause in women.
  • Pregnancy: the baby needs extra iron.
  • Diet low in iron, especially with lots of tea with meals.
  • Blood loss from the gut: ulcers, piles, regular use of painkillers such as ibuprofen or aspirin, and sometimes bowel cancer.
  • Worm infections in children.

Normal haemoglobin levels

GroupAnaemia if haemoglobin (Hb) is below
Adult men13 g/dL
Adult women (not pregnant)12 g/dL
Pregnant women11 g/dL
Children 6 months – 5 years11 g/dL

These are the WHO cut-offs. A ferritin test shows your iron stores, and can be low even before haemoglobin falls.

Iron injections or drips (such as Venofer or Ferinject) are given in hospital or clinic when tablets don’t work, can’t be tolerated, or iron is needed quickly, for example late in pregnancy.

How to take iron tablets so they work

  1. Take iron 1 hour before food, or with a glass of orange juice. Vitamin C helps absorption. If it upsets your stomach, take it with a small amount of food.
  2. Avoid tea, coffee, milk and antacids for 2 hours around your dose.
  3. Keep iron away from children. An iron overdose is dangerous for a child.
  4. Recheck your blood after about 4 weeks. Haemoglobin should be rising.
  5. Continue for around 3 months after levels are normal, as your doctor advises.

Side effects: dark or black stools (harmless), constipation, diarrhoea, nausea or stomach ache. Taking iron every other day can reduce side effects for some people; ask your doctor.

Diet helps prevent a deficiency coming back. See Iron-Rich Foods That Actually Fix Low Hemoglobin.

Frequently asked questions

What are the first signs of iron deficiency?

Tiredness, shortness of breath on light activity and pale skin are usually first. Many people put it down to a busy life, so a blood test is worth it if it persists.

How long does it take to increase haemoglobin with iron tablets?

Haemoglobin usually starts rising within 2 to 4 weeks, and often returns to normal in 2 to 3 months. Iron stores take longer to refill.

Which is the best iron tablet in Pakistan?

The best one is the one you can take regularly without side effects. Ferrous salts and iron complexes both work; your doctor chooses based on your levels and how well you tolerate it.

Can I drink tea with iron tablets?

Avoid it within 2 hours of your dose, because tea reduces how much iron your body absorbs.

Why are my stools black on iron tablets?

Black or dark green stools are a normal effect of iron. Tarry, sticky black stools with weakness or dizziness could be bleeding; get them checked.

Can eating mitti (clay) be a sign of anaemia?

Yes. Cravings for clay, ice or chalk (pica) are a known sign of iron deficiency, especially in children and pregnant women.
