Most people with high blood pressure have no symptoms at all, which is why it’s called a “silent” condition. When the symptoms of high blood pressure do appear, they can include headaches, blurred vision, nosebleeds, shortness of breath or chest pain, and they usually mean the pressure is very high. High blood pressure is a reading of 140/90 or more at a clinic, or 135/85 or more at home. The only way to know is to check it.

Key points

  • Get your BP checked at least once a year after age 40, or more often if it’s raised.
  • Headache is not a reliable sign. Many people with very high BP feel completely fine.
  • Cutting salt, losing weight, walking daily and stopping smoking can lower BP noticeably.
  • BP medicines work only while you take them. Don’t stop when readings improve.
  • Very high BP with chest pain, weakness or confusion is an emergency.

Symptoms of high blood pressure

High blood pressure usually causes no symptoms. Rarely, especially when it’s very high, people notice:

  • Headaches, often at the back of the head
  • Blurred vision
  • Nosebleeds
  • Shortness of breath
  • Chest pain or a pounding heartbeat

Feeling stressed, flushed or “hot” doesn’t reliably mean your BP is high. Many people in Pakistan stop their medicine because they “feel fine”. That is how strokes happen.

Blood pressure chart: normal and high ranges

Reading (mmHg)Category
90/60 to 120/80Ideal for most adults
120/80 to 139/89 (clinic)Above ideal. Lifestyle changes help.
140/90 or higher (clinic), 135/85 or higher (home)High blood pressure
180/120 or higherVery high. Recheck after 5 minutes’ rest and get urgent advice.

The top number (systolic) is the pressure when the heart beats; the bottom number (diastolic) is the pressure between beats. Either being high counts.

Why high blood pressure matters

Over years, high pressure damages the arteries and raises the risk of stroke, heart attack, heart failure, kidney disease and eye damage. People of South Asian background have a higher risk, and high BP often appears alongside diabetes.

Who is at higher risk?

  • Age over 40, or a parent or sibling with high BP
  • South Asian background
  • A salty diet: achaar, namkeen snacks, papad, processed foods and extra salt at the table
  • Being overweight or inactive
  • Smoking or sheesha, and long-term stress
  • Diabetes or kidney disease

How to lower blood pressure

Lifestyle changes that work

  1. Cut salt: cook with less salt, avoid adding salt at the table and limit salty snacks and achaar.
  2. Move more: at least 150 minutes a week of brisk walking or similar activity.
  3. Lose weight if you’re overweight. Even 4 to 5 kg can lower BP.
  4. Eat more fruit, vegetables, daal and whole grains, and less fried food.
  5. Stop smoking and limit caffeine.
  6. Sleep and stress: poor sleep and constant stress keep BP raised.

Medicines for high blood pressure

If your BP stays high, your doctor will prescribe medicine. Common types include ACE inhibitors and ARBs (such as losartan or valsartan), calcium channel blockers (such as amlodipine), thiazide-type diuretics and beta blockers. These are prescription medicines. The right choice depends on your age, other conditions and blood tests. Many people need two or more.

  • Take your medicine at the same time every day, even when you feel well.
  • Don’t stop or change it without speaking to your doctor.
  • Ask a pharmacist before taking painkillers such as ibuprofen, or cold medicines with decongestants. These can raise BP.

Check your blood pressure at home

A home monitor shows how your BP behaves in daily life and whether treatment is working. See our BP machine price guide for models and how to measure correctly.

Frequently asked questions

What are the first symptoms of high blood pressure?

Usually there are none. When symptoms happen, headaches, blurred vision, nosebleeds or chest pain can appear, and they usually mean BP is very high.

What is normal blood pressure for my age?

For most adults, ideal BP is between 90/60 and 120/80 regardless of age. Your doctor may accept slightly higher targets in older age.

Can high BP be cured without medicine?

Mildly raised BP can often be brought down with lifestyle changes alone. Many people still need medicine, especially if BP is high or they have diabetes or kidney disease.

Which medicine is best for high blood pressure?

There is no single best medicine. Your doctor chooses based on your age, other conditions and test results. Common options include amlodipine, losartan and thiazide-type diuretics.

Can stress cause high blood pressure?

Stress raises BP temporarily, and long-term stress can contribute to high BP, especially through poor sleep, diet and smoking.

Is 140/90 dangerous?

It’s the threshold for high blood pressure at a clinic. It isn’t an emergency, but it needs follow-up with your doctor.
