The main medicine for UTI (urinary tract infection) is a short course of antibiotics prescribed by a doctor, with paracetamol for pain and plenty of fluids. Typical symptoms are pain or burning when passing urine, needing to go often and urgently, cloudy or bloody urine and lower tummy pain. Mild bladder infections sometimes clear on their own within a few days, but fever, back pain or shivering can mean the infection has reached the kidneys, which needs urgent treatment.

Key points

  • Don’t take leftover or “general” antibiotics. The wrong antibiotic encourages resistant infections, which are increasingly common in Pakistan.
  • A urine test (urine R/E and culture) helps choose the right antibiotic, especially for repeat infections.
  • Pregnant women, men, children, older adults and people with diabetes should always see a doctor.
  • Cranberry products may help prevent repeat UTIs but don’t treat an active infection.
  • Drink enough water to keep urine pale, and don’t hold in urine.

UTI symptoms in women

Bladder infection (cystitis)Kidney infection: urgent
Pain or burning when peeingHigh temperature, shivering
Needing to pee often, urgently or at nightPain in the back or side, under the ribs
Cloudy, strong-smelling or bloody urineFeeling sick or vomiting
Pain low in the tummyConfusion or drowsiness (especially in older people)

Dark, strong-smelling urine on its own often just means you need to drink more water.

Medicine for UTI: what actually treats it

Antibiotics (prescription only)

A doctor chooses the antibiotic based on your symptoms, pregnancy status, allergies and, ideally, a urine culture. Courses for an uncomplicated bladder infection in women are usually short, often 3 to 7 days depending on the medicine. Take the full course even if you feel better. Divine Medicines supplies antibiotics only against a valid prescription.

Pain relief and self-care

  • Paracetamol (Panadol) for pain and fever, within the label dose.
  • Drink enough water to pass pale urine through the day.
  • Limit coffee, fizzy drinks and very sugary juices while symptoms last.
  • Rest, and use a warm water bottle on the lower tummy for discomfort.

Cranberry for repeat UTIs

Some evidence shows cranberry products may lower the chance of repeat bladder infections in women who get them often. They don’t ease symptoms or cure an active infection. Avoid cranberry if you take warfarin, and check with your doctor in pregnancy.

How to prevent UTIs

  1. Drink plenty of water through the day, more in hot weather.
  2. Don’t hold in urine; empty your bladder fully.
  3. Wipe from front to back after using the toilet.
  4. Pass urine soon after sex.
  5. Wear cotton underwear and avoid very tight clothing.
  6. Avoid perfumed soaps, douches and harsh washes in the genital area. A mild, unscented wash or plain water is enough.
  7. If you have diabetes, keep your sugar under control. High sugar makes UTIs more likely.

UTI in pregnancy

UTIs in pregnancy can harm both mother and baby if untreated, and some antibiotics aren’t safe in pregnancy. Pregnant women with any urinary symptoms should see a doctor promptly and only take antibiotics prescribed for them. Doctors also check urine routinely in pregnancy, because infection can be present without symptoms.

Recurrent UTIs

If you get 2 UTIs in 6 months or 3 in a year, see a doctor. You may need a urine culture, an ultrasound, a sugar test, or a prevention plan. Repeated self-treatment with antibiotics bought without a prescription often leads to resistant infections that are harder to treat.

Frequently asked questions

What is the best medicine for UTI?

Antibiotics prescribed by a doctor are the treatment that cures a bacterial UTI. Paracetamol helps the pain, and fluids help flush the bladder.

Can a UTI go away on its own?

Mild bladder infections in otherwise healthy women sometimes clear within a few days. If symptoms are severe, last more than 2 days or include fever or back pain, see a doctor.

How long does a UTI last with antibiotics?

Symptoms usually start improving within 1 to 2 days of the right antibiotic. If they don’t, contact your doctor.

Does cranberry juice cure a UTI?

No. Cranberry products may help prevent repeat infections in some women, but they don’t treat an active infection. Juices also contain a lot of sugar.

Is it safe to take antibiotics for UTI in pregnancy?

Some antibiotics are safe in pregnancy and some are not. Only take the one your doctor prescribes for you.

Why do I keep getting UTIs?

Common reasons include not drinking enough, holding in urine, sex, menopause, diabetes and incomplete treatment. Recurrent UTIs need a doctor’s review and a urine culture.
